Every visit is unique and every protocol has phases.
Patients in pain will find it hard to follow the traditional method of physical therapy where you are asked to perform exercises (many times with little or no supervision by the actual PT). So, in acute cases the spasms and lack of mobility have to be addressed. Soft tissue techniques are frequently used as are advanced stretching techniques. Manipulations are done when the patient can comfortably tolerate them, never before.
Later visits slowly transition from passive to active. Re-activation is crucial to lasting care and the focus shifts to evaluating the patients' activities (household, work and exercise) and alterations are made to prevent further damage and aggravation.
All care is extremely specific and individualized.
